There are three different LEGO Polybags on offer when spending various amounts on selected themes. To qualify you’ll need to spend £35 ($40/40€) on each of these themes, LEGO City & Creator 3-in-1, LEGO Friends or LEGO Disney sets. The chosen theme will get you the corresponding LEGO Polybag.
